We need a way for people to be able to fill out this standard health questionnaire (attached): The Promise 29 + 2 (we will likely be adding a few questions to this), and then have their results calculated automatically. IMPORTANT – Results will be based on multiple assessments. For example, filling out the form just once will only provide a base-line. We need a way for people to fill this out multiple times and see their changes over time. So the form needs to recognize the person (e.g. by name or by email address), retain there information, and then deliver the results of the progress (e.g. change in data since the first assessment, second assessment, etc). I'm hoping this can be done simply using off the shelf forms like formsort; jotform; typeform; or google docs. Let me know if you have any questions.
