Zoho Survey Work or another Database for quizzes – Upwork
We are a 501c3 non profit that deals with mental health. We focus specifically on a little know condition called Avoidant Personality (Avp) Avp includes Major Depression, Severe Social Anxiety, Extreme Sensitivity to rejection, Extreme Sensitivity to criticism and a lot more.
We have a brand new web site that we hope will be completed soon. One of the main features of this web site is that people will be able to take quizzes that will determine several things.
If they have Avoidant Personality
If their partner has Avoidant Personality and a few other quizzes
We are currently using Zoho Survey for these quizzes.
Here is how this should work
• Visitor takes quiz “Do I have Avoidant Personality”
• Before taking the quiz, user has to register or sign up
• After completing the quiz, user should get instant results and recommendations
• If possible, I would like for them to get instant results on the web site
• If not possible, or in addition to, I want them to get the results sent to their email address
• These results should say “You have an X percentage chance that you have Avoidant Personality
• Then offer the user recommendations based on this percentage
• Bonus, we also need to gather data for research on all the answers each visitor gives also with other demographic data.
Please give me a flat rate quote for the project both with and without the bonus bullet point.
